Mark Emig, MD

Dr. Emig is a specialist in medical and surgical treatment of the vitreous and retina, diabetic eye disease, and cataracts. Born in Memphis, he was raised in the Midwest. He holds a
bachelor of arts degree with distinction from the University of Kansas and a medical degree from the University of Kansas Medical Center. Following an internship at St. Joseph's Hospital in
Phoenix, Dr. Emig served his ophthalmology residency at the University of Kansas Medical Center. He completed his formal training with a fellowship in vitreoretinal disease at the University
of Nebraska Medical Center and Midwest Eye Care. Dr. Emig joined Midwest Eye Care in 1994.
Dr. Emig is board certified by the American Board of Ophthalmology. Dr. Emig is Midwest Eye Care's medical director for Mission Cataract, an annual event where patients without the financial
means to afford cataract surgery are selected for free surgery. He also provides volunteer care for the Indian-Chicano Health Center, Council Bluffs Community Health Center and Hope
Medical Outreach Coalition.

bachelor of arts degree from the University of Nebraska at Lincoln and her doctor of optometry degree from Pacific University in Portland, Oregon. While at Pacific University, Dr. Geist
completed an externship at the Portland VA Medical Center and was active in contact lens research. Dr. Geist worked in a rural-based vision care practice in South Dakota immediately after
her graduation in 1996 and joined the Omaha offices of Midwest Eye Care in 1997.
Dr. Geist is licensed for the practice of optometry by the state examining boards of Nebraska, South Dakota and Oregon. She is a member of both the Nebraska Optometric Association and the
American Optometric Association. She currently serves as a board member for the Nebraska Optometric Association and lobbies the Nebraska Unicameral on children's eye health issues. She
provides volunteer services for the Nebraska Optometric Association "See to Learn" program.

Dr. Grennan is a specialist in cataract and laser surgery, and diagnostic and surgical eye care.
A native of Lincoln, Nebraska, she received a bachelor of arts degree from Pomona College in Claremont, California and a medical degree from the University of Nebraska College of Medicine. Dr. Grennan completed an internship in internal medicine and a residency in ophthalmology at the University of Nebraska College of Medicine.
Dr. Grennan joined Midwest Eye Care after completing her residency program in June 2003. Dr. Grennan is board eligible for the American Board of Ophthalmology; she will sit for oral and written board certification exams in 2003 and 2004.
John Griffiths, MD

Dr. Griffiths is a specialist in ophthalmic plastic and reconstructive surgery, cataract surgery and laser surgery. Born in Auburn, Nebraska, Dr. Griffiths completed his medical degree
at the University of Nebraska College of Medicine in Omaha. Following an internship at the U.S. Naval Hospital in Portsmouth, Virginia, he completed his ophthalmology residency at the
University of Nebraska College of Medicine. Dr. Griffiths practiced as a solo practitioner from 1971 until 1987 when he merged his practice with Midwest Eye Care.
Additional training included a preceptorship in ophthalmic plastic surgery at Wilmer Institute in Baltimore and a fellowship in ophthalmic plastic surgery at Manhattan Eye and Ear Institute
in New York. Dr Griffiths is board certified by the American Board of Ophthalmology and is a Fellow of the American College of Surgeons and the American Society of Ophthalmic Plastic and
Reconstructive Surgery.

Dr. Hanks is a specialist in cataract and laser surgery, and the medical and surgical treatment of eye disease. Dr. Hanks was born in Idaho and received his undergraduate training at
Northeast Missouri State University and Brigham Young University. He received his doctor of osteopathy degree from Kirksville College of Osteopathic Medicine and performed an internship at
the William Beaumont Army Medical Center in El Paso. He completed his residency at Walter Reed Army Medical Center in Washington, D.C. He acted as staff ophthalmologist and flight surgeon in
the U.S. Army for four years before joining Midwest Eye Care in 1989.
Dr. Hanks is board certified by the American Board of Ophthalmology. He is a former chief of medical staff at a local hospital and serves on various hospital committees.

Courtney Hellman, MD (Curriculum Vitae)
Dr. Hellman is serving patients at Midwest Eye Care from July 2003 to June 2004 through the vitreoretinal fellowship program co-sponsored by Midwest Eye Care and the University of Nebraska Medical Center. Dr. Hellman's practice during this period will be primarily limited to co-managing, with Dr. Madson, the care of patients with retinal disease.
Dr. Hellman received her medical degree from the University of Nebraska Medical Center (UNMC). After completing a one-year internship in internal medicine and a three-year residency program in ophthalmology at UNMC, Dr. Hellman completed a glaucoma fellowship at UNMC in June 2003. Dr. Hellman will be the first ophthalmologist in the area with fellowship training in both glaucoma and vitreoretinal disease and surgery. She will join Midwest Eye Care's professional staff in July 2004.
Kathryn Hodges, MD

Dr. Hodges is a specialist in cataract and laser surgery, and diagnostic and surgical eye care. She has a particular interest in treating childhood vision problems. A native of
Independence, Missouri, she received a bachelor of science degree from the University of Missouri at Columbia and a medical degree from the University of Missouri College of Medicine in
Columbia. Dr. Hodges completed an internship and ophthalmology residency at the University of Texas Southwestern Medical Center in Dallas, and joined Midwest Eye Care in 1990.
Dr. Hodges is board certified by the American Board of Ophthalmology and is past chairman of the surgery department at a local hospital. She provides volunteer services for the Indian
Chicano Health Center, Hope Medical Outreach Coalition, and the Nebraska Optometric Association "See to Learn" program.

Everett Madson, MD

Dr. Madson is a specialist in diabetic eye disease; glaucoma treatment; retina, vitreous and laser surgery; and cataracts and implants. A native of Omaha, he received a bachelor of arts degree from the
University of Nebraska at Lincoln, a master of science degree from the University of Iowa in Iowa City and a medical degree from Case Western Reserve Medical School in Cleveland. Following
an internship at Cleveland Metropolitan General Hospital, Dr. Madson completed his ophthalmology residency at the University of Iowa Hospitals. He joined Midwest Eye Care in 1971.
Dr. Madson serves as director of retinal fellowship program for the University of Nebraska College of Medicine in Omaha, is a consultant to area managed care organizations, and is the past
chief of surgery for a metropolitan hospital. He is board certified by the American Board of Ophthalmology and is a Fellow of the American College of Surgeons. He provides volunteer
services to the Hope Medical Outreach Coalition and is a long-time consulting member to United Health Care for "Diabetes Care 2000."

Martin Mizener, MD (Curriculum Vitae)
Dr. Mizener is a specialist in glaucoma treatment and surgery, anterior segment disease, and cataract surgery. A native of Charleston, Illinois, Dr. Mizener received his bachelor of science degree from Eastern Illinois University and his medical degree from the University of Illinois College of Medicine. Following an internship in transitional medicine at the Methodist Medical Center in Peoria, Dr. Mizener completed his ophthalmology residency training at the University of Minnesota. Dr. Mizener was on staff at the University of Iowa Department of Ophthalmology from 1993 to 1995, and served as interim director of the Comprehensive Ophthalmology Service in the Department of Ophthalmology from 1993 to 1994. He completed his formal medical education with fellowship training in glaucoma at the University of Iowa.
In addition to his work and faculty experience at the University of Iowa, Dr. Mizener served as a glaucoma specialist and general ophthalmologist at an Omaha ophthalmology practice from 1996 to 2003. He joined Midwest Eye Care in May 2003.
Dr. Mizener is board certified by the American Board of Ophthalmology, and is also an examiner for the American Board of Ophthalmology. Dr. Mizener is a member of the board of directors for Prevent Blindness Nebraska.
Peter Whitted, MD, JD

Dr. Whitted is a specialist in external ocular disease, anterior segment, cataract and laser surgery, glaucoma and LASIK/vision correction surgery. A native of Omaha, he received a bachelor of
science degree from the University of Nebraska at Lincoln and a law degree from the University of Nebraska College of Law, Lincoln. Dr. Whitted went on to earn a medical degree from the
University of Nebraska Medical Center in Omaha. Following a pathology internship at the University of Colorado College of Medicine in Denver, Dr. Whitted completed his ophthalmology
residency at Baylor College of Medicine, Cullen Eye Institute, Houston. Dr. Whitted joined Midwest Eye Care in 1984.
Dr. Whitted serves as a clinical instructor at the University of Nebraska College of Medicine, the chief of surgery for a metro area hospital and the medical director of the Nebraska Lions
Eye Bank. He is board certified by the American Board of Ophthalmology and is a member of the Nebraska Bar Association. He currently serves as the Treasurer of the Metro Omaha Medical
Society and is past president of the Nebraska Academy of Ophthalmology. Dr. Whitted provides surgical services each year for Mission Cataract USA, and also provides volunteer services
to Hope Medical Outreach Coalition, Indian Chicano Health Center and Prevent Blindness Nebraska.
