In order for us to ensure a timely response, we request that you contact us by telephone if you have a question regarding your medical condition, prescription renewals or medical care in general. If you simply need to contact need us regarding a change in address, a question about your bill or other basic questions, you can e-mail us at jslagle@midwesteyecare.com.

Regular mail
Our main office is located at 4353 Dodge Street, Omaha, NE 68131. In order to ensure timely routing of the mail, be sure to include the name or position of intended recipient. For example, if you want copies of your medical records mailed to another physician, include the title "Medical Records Clerk" above our address. If you have an issue to address with either our administrator, Jay Slagle, or our medical director, Dr. Michael Halsted, be sure to include their names in the address.
